Schedule Request Form

The above form will allow MHS students to request course changes for the 2018-2019 school year.  Schedule changes will only be looked at because of:  Not scheduled into a correct course (example:  signed up for Physics but has Environmental Science listed) or not signed up for the correct level (On-level, Honors, or AP).  If the student needs multiple classes updated they must fill out the form for EACH request.  No late submissions will be accepted after the deadline of August 24th at 4 pm.  Counselors will reach out to the student when the schedule change has been processed.

This form is only to request a schedule change.  We will make efforts to honor requests however a request does not guarantee a schedule change.  No schedule requests will be made for a different instructor or time of day of class.  We will attempt to have all schedules updated by the end of the school day on Tuesday, August 28th.
